Urogynecology Center

The Urogynecology Center of the Clinic of Gynecology, Obstetrics, and Neonatology, First Faculty of Medicine, Charles University and General University Hospital in Prague, provides comprehensive care for women with lower urogenital tract dysfunction. Its main focus is on women with stress urinary incontinence, pelvic organ prolapse, overactive bladder, and monitoring and care for women after childbirth injuries.

Do you have trouble controlling your urine?

We address urinary incontinence issues in our specialized urogynecology clinic. If you experience any problems with urine control, such as during coughing, sneezing, sports, or similar situations, schedule a consultation and examination at our clinic. The same procedure applies to issues with frequent urination or urgency (unpleasant urge to urinate).

Do you feel a prolapse of the uterus or vagina?

If you feel pressure or prolapse in the vaginal area, whether involving the uterus or after surgery, please schedule an appointment at our specialized urogynecology clinic.

Do you have problems in the vaginal and perineal area after childbirth?

If you have problems in the vaginal or perineal area after childbirth, please schedule an appointment at our specialized urogynecology clinic.

At our perineal clinic, we specialize in diagnosing postpartum injuries of the pelvic muscles and, if necessary, the anal sphincter. In cases where proper healing has not occurred or you are experiencing

Do you have chronic vaginal infections?

If you have long-term and recurring vaginal infections, you can schedule an appointment at our specialized urogynecology clinic, preferably after consulting with your attending gynecologist or based on their recommendation.

Do you have nonspecific pain in the pelvic area, vaginal opening, or vagina?

If you have long-term, unexplained discomfort in the vagina, vaginal opening, or pelvic area (such as pain during intercourse, burning at the vaginal opening, pain after urination, pain during bowel movements, and other symptoms), you can schedule an appointment at the specialized clinic for pelvic neuropathic pain. It is best to do this after consulting with your attending gynecologist or based on their recommendation, once other possible causes of the pain have been ruled out and your doctor suspects neuropathic pain.

What Else Do We Treat?

The center also addresses complications from previous surgeries – removal of eroded slings, removal of slings from the urethra or bladder, and specialized techniques for sling removal in cases of neuropathic pain caused by sling placement.

  • In cooperation with urologists, the center performs reconstruction of vesicovaginal fistulas (either laparoscopically or vaginally).
  • For prolapse treatment, the full spectrum of classic vaginal reconstructive surgeries is performed, including suspension to the sacrospinous ligaments; in very high-risk patients, colpocleisis is also performed. A large number of patients also undergo laparoscopic sacrocolpopexy.
  • The center focuses on managing complications from previous prolapse surgeries, including the removal of implanted materials.
  • For overactive bladder (OA B) syndrome, comprehensive diagnostics are available, including botulinum toxin application. For complicated cases, sacral neuromodulation is also offered.
  • At the perineal clinic, women with extensive postpartum injuries are monitored. Surgical options include repair of persistent fecal incontinence with secondary suturing of the external anal sphincter.

Perineal remodeling after extensive injuries is also performed. For women with persistent fecal incontinence, the center also provides indication for sacral neuromodulation.

Collaboration with Other Specialties

The center performs comprehensive clinical diagnostics for women with suspected neuropathic issues in the pelvic and lower urogenital tract. Adequate diagnosis and treatment require collaboration with pain specialists (specialized anesthesiologists), urologists, neurologists, physiotherapists, and radiology (see diagram).

  • A large portion of the complaints are functional in nature and are addressed through physiotherapy.
  • An integral part of treatment is collaboration with a pain specialist and appropriately indicated pharmacological treatment for chronic pain.
  • If a neurological cause is suspected, patients are referred for a consultative neurological examination with a collaborating neurologist.
  • If a urological cause is suspected (painful bladder syndrome), patients are referred to a urology clinic specialist who manages these types of complaints.
  • If pelvic congestion syndrome is suspected, patients are referred to a collaborating interventional angiologist for embolization.

 

Laparoscopic Pudendal Nerve Release: If conservative treatment fails and all other possible causes of neuropathic pain are excluded, laparoscopic pudendal nerve release is performed in patients with pudendal neuralgia.

Neuromodulation Treatment: For complicated cases, neuromodulation treatment may be considered in collaboration with an interventional pain specialist. At our center, we perform direct implantation of neuromodulation electrodes onto the affected nerve (pudendal nerve).

  • An integral part of treatment is the possibility of consulting a clinical psychologist at any stage of treatment.
